Consent to the processing of personal data

I hereby inform you that:

1. The Administrator of your personal data is Unbreakable You Angelika Urbanek — hereinafter referred to as the Administrator.
2. The Administrator carries out processing operations of your personal data for the purpose of organizing and providing psychological and psychotherapeutic services.
3. Your personal data (and those of the person under your legal care*) are covered by the professional secrecy of a psychotherapist. They will be processed for the purpose of maintaining psychological documentation, conducting sessions in the office and online, and issuing invoices. With the exception of a written court order to lift the psychotherapist’s professional secrecy or situations of direct or indirect threat to life or health, which must be reported to the appropriate services, no other external entity is a recipient of your data — except for the situation of using online session services via a communicator (e.g., Google Meet, Skype, WhatsApp, or another). The Administrator undertakes to perform the service with respect for the privacy of the conversation via the selected communicator, but does not guarantee data security in the internet space.
4. The basis for processing your personal data is Article 6(1)(b) and Article 9(2)(h) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) — OJ EU L.2016.119.1 as amended.
5. Providing personal data is voluntary, but necessary to conclude the contract. In the event of failure to provide data, it is impossible to conclude a contract for the provision of psychological consultation services and therapeutic sessions, as well as any other documents containing personal data and any activities related thereto.
6. You have the right to:
   • access your data and receive a copy thereof,
   • request the rectification (correction) of your data, their erasure, or restriction of their processing,
   • data portability,
   • object to the processing of personal data,
   • lodge a complaint with a supervisory authority dealing with personal data protection in the Member State of your habitual residence, place of work, or place of the alleged infringement (in Poland, this is the President of the Personal Data Protection Office, ul. Stawki 2, 00-193 Warsaw),
   • withdraw your consent to the processing of personal data at any time.
7. Your personal data are not subject to automated decision-making, including profiling.
8. Your personal data will be stored by the Administrator for a period of 2 years from your last visit. In the event of failing to obtain consent for further data storage, they shall be destroyed.

* applies to situations where the Patient is a person under legal care.

Consents related to the appointment booking system. Mandatory consents are necessary to make a reservation and provide services; voluntary consents can be withdrawn at any time.
