GDPR Consents
Consents connected with the appointment booking system. The required consents are necessary in order to make a booking and receive the services; the optional consents may be withdrawn at any time.
1. Provision of online psychological / psychotherapeutic services
I consent to the processing of my personal data, including special category data concerning health that I disclose during online psychological or psychotherapeutic consultations, by Unbreakable You Angelika Urbanek, for the purpose of concluding and performing a contract for the provision of psychological services and of keeping the records relating to those services. I am aware that the basis for this processing is Article 6(1)(b) and Article 9(2)(h) GDPR, and that refusing to give this consent makes it impossible to book an appointment and to receive online psychological services.
2. Organisational contact (SMS, e-mail, telephone)
I consent to being contacted by Unbreakable You Angelika Urbanek using the telephone number and e-mail address I have provided, in matters relating to the confirmation, rescheduling or cancellation of an appointment, and in order to pass on organisational information concerning the provision of online psychological services. I am aware that this consent is necessary for the proper performance of the contract.
3. Supervision
I consent to information concerning the course of my therapy being used in clinical supervision conducted by a psychotherapist cooperating with Unbreakable You Angelika Urbanek, solely for the purpose of ensuring the quality and safety of the therapeutic process. This consent covers anonymised information only, without disclosure of data identifying me. The consent is voluntary and may be withdrawn at any time.
4. Marketing / newsletter
I consent to the processing of my personal data by Unbreakable You Angelika Urbanek for marketing purposes, in particular in order to send information about new services, educational materials, events or content relating to the activity of the practice. I am aware that this consent is voluntary and may be withdrawn at any time, without affecting the lawfulness of processing carried out before its withdrawal.
5. Use of ICT tools (e.g. Google Meet)
I consent to the processing of my personal data in connection with the use of the ICT tools employed by Unbreakable You Angelika Urbanek, in particular Google Meet, in accordance with the rules set out in the privacy policies of those providers. I am aware that data may be transferred outside the European Economic Area on the basis of standard contractual clauses, in accordance with Article 46 GDPR. The consent is voluntary and may be withdrawn at any time.
