They will be processed for the purpose of maintaining psychological documentation, conducting sessions in the office and online, and issuing invoices. With the exception of a\u00a0written court order to lift the psychotherapist&#8217;s professional secrecy or situations of direct or indirect threat to life or health, which must be reported to the appropriate services, no other external entity is a\u00a0recipient of your data \u2014 except for the situation of using online session services via a\u00a0communicator (e.g., Google Meet, Skype, WhatsApp, or another). The Administrator undertakes to perform the service with respect for the privacy of the conversation via the selected communicator, but does not guarantee data security in the internet space.<\/li>\n\t\t\t<li>The basis for processing your personal data is Article 6(1)(b) and Article 9(2)(h) of Regulation (EU) 2016\/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95\/46\/EC (General Data Protection Regulation) \u2014 OJ EU L.2016.119.1 as amended.<\/li>\n\t\t\t<li>Providing personal data is voluntary, but necessary to conclude the contract. In the event of failure to provide data, it is impossible to conclude a\u00a0contract for the provision of psychological consultation services and therapeutic sessions, as well as any other documents containing personal data and any activities related thereto.<\/li>\n\t\t\t<li>You have the right to:<ul class=\"mt-3 list-disc space-y-2 pl-6 text-[16px] leading-[1.6] text-[#4a4034]\"><li>access your data and receive a\u00a0copy thereof,<\/li><li>request the rectification (correction) of your data, their erasure, or restriction of their processing,<\/li><li>data portability,<\/li><li>object to the processing of personal data,<\/li><li>lodge a\u00a0complaint with a\u00a0supervisory authority dealing with personal data protection in the Member State of your habitual residence, place of work, or place of the alleged infringement (in Poland, this is the President of the Personal Data Protection Office, ul. Stawki 2, 00-193 Warsaw),<\/li><li>withdraw your consent to the processing of personal data at any time.<\/li><\/ul><\/li>\n\t\t\t<li>Your personal data are not subject to automated decision-making, including profiling.<\/li>\n\t\t\t<li>Your personal data will be stored by the Administrator for a\u00a0period of 2 years from your last visit. Provision of online psychological \/ psychotherapeutic services<\/h3>\n\t<p class=\"mt-3 text-[16px] leading-[1.65] text-[#4a4034]\">I\u00a0consent to the processing of my personal data, including special categories of data concerning health, disclosed by me during online psychological or psychotherapeutic consultations, by Unbreakable You Angelika Urbanek, for the purpose of concluding and executing a\u00a0contract for the provision of psychological services and maintaining documentation related to these services. Organizational contact (SMS, e-mail, phone)<\/h3>\n\t<p class=\"mt-3 text-[16px] leading-[1.65] text-[#4a4034]\">I\u00a0consent to being contacted by Unbreakable You Angelika Urbanek using the phone number and e-mail address provided by me in matters related to confirming, changing, or canceling the appointment date, as well as for the purpose of communicating organizational information regarding the provision of online psychological services. I\u00a0am aware that this consent is necessary for the proper performance of the contract.<\/p>\n<\/div>\n\t\t<div class=\"mt-5 rounded-2xl border border-[#e5ddd2] bg-[#fffdfa] p-6\">\n\t<div class=\"flex flex-wrap items-center gap-3\"><span class=\"inline-block rounded-full border border-[#c9bcac] px-3 py-1 text-[14px] font-semibold uppercase tracking-[0.08em] text-[#6a6054]\">Voluntary consent<\/span><span class=\"text-[14px] italic text-[#6a6054]\">if the practice uses it<\/span><\/div>\n\t<h3 class=\"mt-3 text-[24px] tracking-[0.02em] leading-[1.15] text-[#32271A]\">3. The consent covers only anonymized information, without disclosing my identifying data.
